For general information, teachers may include the date, teacher's name, number of students, level of the class, teaching materials, context and focus of the lesson, students' objective, teacher's aim, and problems that may arise and their solutions for students and teachers. In the second part of the lesson plan, you should include the procedures, phase, timing, and interaction between the teacher and the students in every activity.
