Some EFL teachers may be in a context or culture where coursebooks are practically unavailable, so will have to 'invent their own wheel' or use pooled homespun resources if at least there are any of these at their centre. Any coursebook that is available should be one's slave rather than one's master: the art lies in knowing when to trust and use it, and when circumstances suggest supplementing or circumventing it with more fitting material of one's own customised devising. Realien can be very motivating (more usually with more advanced students) since they have the ring of authenticity and put students, in a sense, on their aspired par with the original native audience. One's own materials can be edited or adapted in the light of experience (worksheet masters, PowerPoint or whatever) and, if lucidly conceived and well-made, can justify the effort spent on their development. Students often prefer to see the teacher has made such an effort for them (unless ~ for instance ~ the artwork / typography are so grotty as to hamper, rather than helping, understanding and communication!).
