I doubt I was the only one to be relieved when 'Lesson 2' came on! The tutor had carefully made practically every mistake in the book 1st time round. Improvements later on included: learning students' names (& being open & humble when he had any trouble pronouncing them); fun game activities introduced clearly, often in monosyllabic instructions and with mime; setting pacy timescales; involving all students; broadening the language later to introduce (or recap) other related structures ('Why can an X do Y; what has it got?' etc); use of both sides of paper rather than screwing it up & chucking it away; not sitting-out during students' individual work, nor leafing boredly through an apparently out-of-date dictionary. In Lesson 2 he was patient, good-natured and encouraging, not swamping student with obscure futile repetition of technical LL jargon; he offered suggestion and reinforcement without repeating-back nor 'too much teacher talk', nor was he tetchily dismissing the material as being 'very easy' any more. Altogether a vast improvement, though the 'perfect lesson' is ever-elusive.
